How to prepare for a plan review

Plan reviews happen at least once a year. They are your opportunity to update your goals, report progress, and request changes to your funding.

Gather your documents

Before the review, collect:

  • Invoices and receipts from the past year
  • Progress reports from your therapists
  • Any hospital discharge summaries or new diagnoses
  • A list of supports that worked well and ones that did not
  • Think about your goals

    Your goals drive the plan. Consider:

  • What did you achieve in the past year?
  • What new goals do you have?
  • Have your circumstances changed?
  • Be specific about funding needs

    If you need more funding in a category, bring evidence. For example:

  • "I used all my therapy budget by March because I started weekly physio."
  • "I need a new wheelchair and the quote is $4,200."
  • You can bring support

    You can have a family member, advocate, or support coordinator attend with you. You do not have to do it alone.
