Support Coordination

Support coordination is about making your NDIS plan work for you. Our coordinators help you understand your budget, find the right providers, resolve issues, and prepare for plan reviews. We take the confusion out of the NDIS so you can focus on your goals — and we build your confidence to manage your own supports over time.

What this looks like in practice

  • Plan interpretation, budget tracking, and spending guidance
  • Provider research, referrals, and contract negotiations on your behalf
  • Crisis resolution, escalation support, and urgent plan variations
  • Preparation for plan reviews with evidence and goal documentation
  • Building your skills and confidence to self-manage over time
  • Liaison between therapists, schools, employers, and other services

Who this service is for

  • New NDIS participants unsure where to start

  • Anyone whose plan has been reduced at review and needs to reassess options
  • Participants with complex needs who need multiple services coordinated

Occupational Therapy

Our occupational therapists help you build the skills and adaptations you need to live as independently as possible. We do not just hand you a report — we work with you until it makes a difference.

What this looks like in practice

  • Daily living skills assessments and individualised intervention plans
  • Sensory profiling and regulation strategies for home, school, and community
  • Home and environment modification recommendations to improve safety and access

Exercise Physiology

Our exercise physiologists design evidence-based exercise programs for participants managing chronic conditions, recovering from injury, or building strength and stamina as part of their disability support. Hospital Transition & NDIS Access

We support participants through safe and coordinated transitions from hospital to home, ensuring continuity of care and reducing the risk of readmission. Our team assists with discharge planning, connecting participants with appropriate supports, arranging in-home services, and coordinating allied health, support workers, equipment, and community services to help individuals settle safely back into their home environment. We also assist individuals in accessing and navigating the National Disability Insurance Scheme (NDIS), including support with eligibility, gathering evidence, completing applications, and connecting participants with the right services once approved. Our goal is to make the process smooth, person-centred, and stress-free for participants and their families.

What this looks like in practice

  • Discharge planning and coordination with hospital and community teams
  • In-home service arrangement including allied health, support workers, and equipment
  • Continuity of care management to reduce risk of readmission
  • NDIS eligibility support, evidence gathering, and application assistance

When you need more than one type of therapy, coordination matters.
